Requester Guide
Patient/Item Request
  1. Dial 78416
  2. Enter your ID number(Any location ID in your area)
  3. Press 1 for patient/item request
  4. Press * and enter patient’s medical record number then #.  Always press the *key before entering the patient's UH number.  Failure to press the *star key and then the UH number, will result in patient not found.
  5. Press 1 to accept name
  6. Press 2 to change
  7. Enter destination then #
  8. Record voice message then #
  9. Press 1 to accept call
  10. Press 2 to change Origin/Destination
  11. Press 3 to re-record voice message
  12. Patient Trac will repeat a Confirmation number.  Press * to make another request.  Remember to always record your confirmation number.

Outpatient Request (Patients not in the system)

  1. Dial 78416
  2. Enter your ID number(The location ID of the room you are in)
  3. Press 4 for an outpatient request
  4. Record voice message then #
  5. Enter four digit destination then press #
  6. Press 1 to accept call
  7. Press 2 to change Origin/Destination
  8. Press 3 to re-record voice message
  9. Patient Trac will assign a Confirmation #. Press * to make another request.  Remember to always record your confirmation number.

Inquiry

  1. Dial 78416
  2. Enter your ID number(Any location ID in your area)
  3. Press 2 for an inquiry:
    Press 1, by your dept
    Press 2, for specific item
    Press 3, using confirmation #
    Press 4, for a specific location
  4. System will give an update:
    Press 1, for scheduled details
    Press 2, for completed jobs
    Press 3, to quit
  5. After hearing scheduled details:
    Press 1, to edit
    Press 2, to cancel
    Press 3, for next call
Specimen Request
  1. Dial 78416
  2. Enter your ID number(Any location ID in your area)
  3. Press 1 for a patient/item request
  4. Press *026#
  5. Press 1 to accept
  6. After the tone, record message and press#.
  7. Press 1 to accept call
  8. Press 2 to change Origin
  9. Press 3 to re-record voice message
  10. Patient Trac will repeat a Confirmation #. Press * to make another request.  Remember to always record your confirmation number.

Out-of-Bed Requests/Back to Bed Request

  1. Dial 78416
  2. Enter ID number 6666 (Transportation dispatch center as origin)
  3. Press 1 for patient/item request
  4. Press * 033#  for Out of Bed 
  5. Press * 035#  for Back to Bed
  6. Press 1 to accept
  7. Enter your area ID number as destination and press #
  8. Record voice message (Patient's name and number of transporters needed) then press #l
  9. Press 1 to accept call
  10. Press 2 to change Destination
  11. Press 3 to re-record voice message
  12. Patient Trac will repeat a Confirmation number.  Press * to make another request.  Remember to always record your confirmation number.

Inpatient Pre-Scheduled Request
  1. Dial 78416
  2. Enter your ID number(Any location in your area)
  3. Press 3 for pre-scheduled request
  4. Press * and enter patient’s medical record number then #.
  5. Press 1 to accept name, or 2 for next patient
  6. Enter the destination then #
       Press 1, for time
       Press 2, for date
       Press 3, for both
  7. Record voice message then press #
  8. Press 1 to accept call
  9. Press 2 to change Origin/Destination
  10. Press 3 to re-record voice message
  11. Patient Trac will repeat a Confirmation #. Press * to make another request.  Remember to always record your confirmation number.

Consult Requests

  1. Dial 78416
  2. Enter your ID number(Any location ID in your area)
  3. Press 1 for patient/item request
  4. Enter the Consult Request Code by pressing the * (star key) and then the number followed by the # (pound sign). 
  5. Press 1 to accept call
  6. To change type of consult press 2
  7. Record voice message then press #.
  8. Press 1 to accept call
  9. Press 2 to change
  10. Patient Trac will repeat a Confirmation number.  Press * to make another request.  Remember to always record your confirmation number.

    * 040# - General Consult
    * 041# - OT Consult
    * 042# - PT Consult
    * 043# - Radiology Consult
    * 044# - Heart Station Consult
    * 045# - Radiation Therapy Consult
    * 046# - Blood Consult
    * 047# - Placenta Consult
Inpatient Auto Request 
  1. Dial 78416
  2. Enter your ID number(The location ID of the room you are in)
  3. Press one of the following auto-request numbers:
      *0, Bring Patient to Me
      *1, Return Patient to Room
  4. Press * and enter patient's medical record number then #.
  5. Press 1 to accept 
  6. Press 2 to change 
  7. Record voice message then #
  8. Press 1 to accept call
  9. Press 2 to change Origin/Destination
  10. Press 3 to re-record voice message
  11. Patient Trac will repeat a Confirmation #. Press * to make another request.  Remember to always record your confirmation number.

Special Notes:

1
.  At any point you can push **# to return to the main menu.

2.  You must record the confirmation number if you wish to perform an inquiry on a specific job.

3.  If the patient's name has not been added to the system and the system asks you to record it, only record the patient's first and last name.
